Making an appointment
Please call (02) 9806 3333 to make a routine appointment with Dr Duckworth.
Office hours are 8am till 5pm Monday to Friday

You may prefer to attend the Emergency Department at

where the staff will be able to provide initial care and contact Dr Duckworth directly for you.

Day of your appointment
Please remember to bring:
- The referral letter from your GP or health practitioner
- X-rays, CT scans and/or MRIs relating to your condition
- Medicare and Health Insurance details
- Workers Compensation details
- A list of your current medications
- Ensure you complete your - PATIENT REGISTRATION
Before your surgery
When the staff at the rooms book you in for surgery, they will discuss with you all the relevant information about the hospital you will be attending.Paperwork will be given to you for the hospital or it can be downloaded online. The hospital staff will call you the day before your surgery to let you know about the time to present for admission and about fasting times and what to bring to hospital.
If you have concerns and questions, Dr Duckworth’s staff are always happy to speak to you over the phone about your admission.
