An injury to the rotator cuff is a common cause of shoulder weakness.
AC joint dislocations occur most commonly when one falls directly on the point of the shoulder.
The shoulder is the most mobile of all joints. Unfortunately it can wear out, which is known as shoulder osteoarthritis.
Subscapularis tendon tears deserve a special mention as they can occur in isolation from the other tendons which make up the rotator cuff.
A fracture of the proximal humerus is more common in elderly individuals with osteoporosis. However it can occur in any age group if you fall the wrong way.
Shoulder surgical procedures
The goal of the surgical repair of the rotator cuff defect is to establish the connection between the torn tendon and the bone.
AC joint reconstructions are reserved for dislocations with unacceptable deformity and collapse of the shoulder joint.
Most proximal humerus fractures are minimally displaced and can be treated non-operatively. Surgery may be necessary in displaced fractures.
I prefer to repair the subscapularis tendon through a small incision 3 to 4 cm long at the front of the shoulder. This is called a mini open repair and gives excellent exposure of the tendon.
The primary purpose of a shoulder arthroplasty is to relieve pain. The secondary purpose is to increase range of motion.
Standard shoulder replacements can’t be used when there are no rotator cuff tendons.
